Problem Report Form

Please fill out the form below to report any problems that occurred before or during operation of IIL instruments. Enter your username and current contact information so we may contact you for problem clarification if necessary. Enter the date and times of the occurrence so we can better troubleshoot the event. Under problem description please indicate operations leading up to the problem, a thorough description of the problem, and what corrective actions (if any) were attempted by the user after the problem occurred.
